Your Rights Under the No Surprises Act
(H.R. 133 – Effective January 1, 2022)
At Anchor Point Behavioral Health, we believe in clarity and empowerment when it comes to your care. As part of our commitment to transparency and ethical practice, we provide a Good Faith Estimate (GFE) of expected costs for services if you are uninsured or not using insurance.
This estimate gives you an outline of the projected fees for your care based on the information available at the time it’s created. It’s intended to help you plan ahead and make informed decisions about your treatment.
What Is a Good Faith Estimate?
A Good Faith Estimate outlines the anticipated cost of your scheduled therapy sessions or requested services. It includes charges for services that are reasonably expected as part of your care, but it may not include any unknown or unexpected costs that arise later due to changes in your treatment needs.
Please know:
You are not required to receive care out-of-network.
You will never be asked to waive your rights to protect yourself from surprise or balance billing.
You may request a Good Faith Estimate at any time—even if you are not scheduling a service yet.
Out-of-Network and Balance Billing
If you choose to see a provider who is not part of your insurance network, you may be responsible for higher out-of-pocket costs. This is known as balance billing, where the provider charges you the difference between their fee and what your insurance reimburses. These charges may not count toward your insurance plan’s deductible or out-of-pocket maximum.
At Anchor Point, we do not engage in balance billing practices. If you are paying out-of-pocket, the full cost of your care will be outlined in your Good Faith Estimate.
Your Right to Dispute a Bill
If you receive a bill that is $400 or more above the amount listed in your Good Faith Estimate, you have the right to dispute the charges.
You can:
Contact us directly to review or revise the bill.
Ask if financial assistance or a payment plan is available.
Initiate a formal dispute resolution process through the U.S. Department of Health & Human Services (HHS).
If you choose to start the federal dispute resolution process, you must do so within 120 days (about four months) of the date on the bill. There is a $25 administrative fee. If the reviewer agrees with your dispute, you will only be required to pay the amount listed in the original Good Faith Estimate.
Timeline for Receiving Your Estimate
Your Good Faith Estimate will be provided within the following timeframes:
If a service is scheduled at least 10 business days in advance: You’ll receive your GFE within 3 business days.
If a service is scheduled at least 3 business days in advance: You’ll receive your GFE within 1 business day.
If you request an estimate without scheduling a service: You’ll receive it within 3 business days of your request.
If your appointment is rescheduled, a new estimate will be issued within the same timeframes.
Important Notes
A Good Faith Estimate is not a contract and does not require you to proceed with services.
It is intended for informational purposes and may be updated if your care plan changes.
All clients at Anchor Point Behavioral Health have the right to receive this estimate upon request, in accordance with federal and state law.
